Living with chronic illnesses like diabetes, hypertension, heart disease, or arthritis can be challenging, but with the right strategies, you can effectively manage these conditions and maintain your quality of life. Stick to Your Treatment Plan

  1. Medication Adherence: Take your medications as prescribed. If you have questions about dosage, timing, or affordability, don’t hesitate to ask your doctor. We need to know if there is a barrier to you adhering to the medications we prescribe.
  2. Health Monitoring: Regularly check and record essential health metrics, such as blood sugar or blood pressure. Keeping a log in between doctor visits can be very helpful for your doctor. Remember to bring the log to your visits. If you note any unusual changes do not wait, call and see your doctor.
  3. Regular Checkups: Be proactive and make regular visits with your doctor. Waiting until you feel “sick” to visit your doctor is letting the disease control you, instead of you proactively managing your health and quality of life.

Embrace a Healthy Lifestyle

  1. Physical Activity: The American Heart Association recommends at least 150 minutes of moderate-intensity aerobic exercise and muscle-strengthening activities weekly. The weekly time and specific activity depends on your specific health status and goals, and can vary from walking, cleaning the house, biking, sports and more. Remember, any physical activity is better than no activity. Work with your doctor to create a safe, effective exercise plan tailored for you.
  2. Healthy Eating: Follow a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Limit foods high in added sugars, sodium, and unhealthy fats (fried foods for example). Summer is a great time to eat fresh produce and enjoy farm fresh salads and vegetables. A nutritious diet is fundamental in managing chronic conditions and preventing complications.
