With unceasing passion for primary care medicine and patient care, Dr Jacob co-founded Primary PartnerCare and leads all strategy, growth and medical operational functions. As a strong believer that physicians should be educated and knowledgeable in all areas of the industry, Dr Jacob has dedicated his life’s work to ensuring that primary care physicians understand the complexities and opportunities of value-based care. Dr Jacob serves as President of Primary PartnerCare Physicians, PLLC and Chief Executive Medical Officer of Primary PartnerCare MSO. Dr Jacob serves as Chair of Primary PartnerCare ACO and Primary PartnerCare Physicians, PLLC Boards of Directors.
Dr Jacob has a long and broad track record working within the industry as an entrepreneur, consultant, medical expert and notably as Chief Medical Officer of Doctors Care Network, one of New York’s first full risk bearing IPAs and fully delegated MSOs with over 6,000 physicians across Long Island and the 5 boroughs. Respectfully described by his colleagues as a "doctor's doctor", Dr. Jacob continues to practice medicine as a Board Certified Internist. He is frequently called upon for his expertise in value-based care and medical group operations.
